Our 120-bed facility is nestled along the Wisconsin River, just minutes from Madison, which offers the scenic beauty and friendliness of the Sauk-Prairie community as well as the finest professional services available.

Established in 1971, Maplewood has a history of being responsive to the changing needs of our aged population. Over the years, we have enhanced our nursing services with respite and hospice care, Alzheimer's/dementia care, psychotherapy, complete rehabilitation services, recreational activities and an in-house pharmacy. In 1999, we opened Maplewood Village, a state of the art assisted living facility located right next door to the skilled nursing facility.
